В организме женщин тестостерон поддерживает анаболические процессы (прежде всего в мышечной ткани), метаболизм и ремоделирование костной ткани.Высокочувствительный и специфичный метод определения тестостерона абсолютно необходим для диагностики патологических состояний как у мужчин, так и у женщин. главная проблема количественного определения тестостерона заключается в перекрестном реагировании с другими субстратами при работе в низком диапазоне значений (менее 10 нмоль/л), что существенно затрудняет его клиническое использование. Физиологическое низкое содержание тестостерона наблюдается у детей до периода полового созревания и женщин. Содержание тестостерона в крови у женщин не превышает 7% от его содержания в крови взрослых мужчин.количественная оценка содержания андрогенов необходима при наличии клинических признаков гиперандрогении (гирсутизм, акне, алопеция) с целью дифференциальной диагностики синдрома поликистозных яичников и андрогенсекретирующих опухолей у женщин [1—4]. Надежный метод количественного определения тестостерона необходим в педиатрической практике при уточнении причин преждевременного полового развития и задержки пубертатного периода, наблюдении пациентов с наиболее распространенной формой врожденной дисфункции коры надпочечников вследствие дефицита 21-гидроксилазы. Точное определение уровня тестостерона необходимо для диагностики распространенного возрастного дефицита андрогенов (PADAM или LOHсиндром): низкое содержание общего тестостерона (об. тестостерон менее 10 нмоль/л) у мужчин в возрасте 40—60 лет регистрируется в 7% случаев, а в возрасте 60—80 лет — в 21% случаев. мониторинг тестостерона необходим при антиандрогенной терапии у больных раком простаты пролонгированными препаратами люлиберина (фармакологическая кастрация).Даже в физиологических условиях уровень общего тестостерона (об.Т) довольно вариабелен, кроме того, возможны лабораторные погрешности на эта-пе проведения измерения. Почти 50—60% об.Т образуют сильную связь с глобулином, связывающим половые стероиды стероиды (гСПС), а 40—50% — с альбумином (легко диссоциирующая связь). Только 1—2% тестостерона циркулирует в свободной форме (св.Т) и вместе со связанным с альбумином тестостероном они формируют биодоступный тестостерон (биоТ).
